About Us

 

Dear Health Care Provider,

I wanted to take a few moments to introduce Case Management Associates to you. 
We are a disease management firm. We are independent contractors and we 
service physicians offices, community health care centers and other health care 
establishments.  We also can be contracted by home health agencies to service 
their clients in their homes to assist with their disease management needs.

Disease management is a new approach to the treatment of certain chronic medical 
conditions and the prevention of serious cardiovascular complications and organ 
damage.

Based on our many years of experience, we have found that many patients 
require 1:1 nursing intervention in order to establish, maintain and continue 
compliance with an established disease management plan.  Case Management 
Associates, LLC (CMA) has developed a model in which patients, with uncontrolled 
diabetes, hypertension, hyperlipidemia and obesity, are offered a wholistic 
approach to disease management.

" CMA's program begins with a contractual agreement with a health care agency, 
community health care center or Primary care center.  Clients are then enrolled into 
the disease management program.  A sound relationship with the patient's Primary 
Care Provider is essential to the success of the program.  This will help facilitate 
changes in the patients medical management there by meeting their disease 
management goals."
