Services: Disease Management & Education

CMA can be an asset to your health care facility by: Offering vital "1 on 1" nursing disease management and education services, monthly
reports to assess patient progress, quarterly reports to assess the success of the program, small group classes that allow patients
to empower each other, A1C monitoring, and all educational materials.

Case Management Associates offers Primary Care Providers and their patients disease management and education services through the following methods:

1. Primary Care Providers or Health Care establishments may enter into a contractual agreement with Case Management Associates.
Through this agreement, CMA will come to you!! We will see your patients, in your office, at your convenience.

2. We are available for 4 hour days and 8 hour days, contractual fee(s) will apply. This is a great option for those Primary Care Providers who realize the importance of having an experienced RN Disease Manager available in their offices.

3. We also offer on-site diabetes classes taught by a Certified Diabetes Educator.
